Do I need to complete a new ACH debit form if if want to change my contribution frequency? If you have a current ACH Debit Form, yes, you will have to update the form. If your contributions are being deducted automatically from your payroll, then any changes will follow your plans frequency to change contribution.

Does the name on my bank document need to match my account name? Yes. The name on the bank document must match the name on your account. If your name has changed due to marital status or other reasons, you will need to update your records before submitting the document.
